Jazmine’s Journey is donating feeding tube covers and port covers to pediatric cancer patients currently in treatment. The pictures on the Zeffy Form are the ones we currently have on hand. How to Request a Cover:
1️⃣ Choose ONE cover you would like to receive.
2️⃣ Email documentation proving current treatment to info@jazminesjourney.com.
Accepted documents include:
- A letter from a social worker
- A treatment plan that includes the child’s name
3️⃣ Once verified, you will receive an email confirming approval and an estimated shipping date.

Requests without proper documentation will not be processed.
